About Divine Embrace Health Services
Divine Embrace Health Services, LLC is a home and community-based services (HCS) provider that is certified by the Department of Aging & Disability Services (DADS).
The HCS Program provides community-based services and supports to eligible persons as an alternative to the intermediate care facilities for persons with Mental Retardation or Related Condition (ICF/MR) Program.
Services are available to persons living in their own home and living with their family or a paid companion.
Services may also be provided to persons living in a small group home where no more than four persons may reside.
Non-HCS private pay is accepted as well. We strive to provide the highest quality of services. We also have the most dependable, skilled, and qualified caring and competent staff for our clients.

SERVICES
INDIVIDUALIZED SKILLS AND SOCIALIZATION (ISS)
Each client is provided the oportunity to participate in a day activity, pre-vocational or vocational setting six (6) hours per day, five (5) days per week as determined by individual needs.
NURSING SERVICES
Provided by an LVN or RN to assist with monitoring of clients health status and consult with medical professinals regarding medication changes and help the client understand their medical needs
RESIDENTIAL SUPPORT/
SUPERVISED LIVING
SERVICES
Supervised Living is provided by a residential assistance staff who provides services and supports as needed and is present in the residence and able to respond to the needs of individuals during normal sleeping hours.
FOSTER/ COMPANION CARE SERVICES
Clients in aresidence will be provided with direct personal assistance with activities of daily living (grooming, eating, bathing,dressing, and personal hygiene). Assisting with meal planning and preparation, securing and providing transportation, assistance with housekeeping, assistance with mobilty, etc.
DENTAL TREATMENT
SERVICES
Dental treatment will be provided as determined by individual needs and is delivered in compliance with the HCS Program Service Definitions
SPEECH & LANGUAGE
PATHOLOGY
One of the specialized therapies provided by appropriately liscensed or certified professionals
RESPITE
Includes assisting with training in self help and indepnedent living skills, provision of room and board, support on a 24-hour increment or any part of that increment for no more than 30 days.
BEHAVIOR/
PSYCH SUPPORT
Specialized therpies included are occupational therapy services, physical therapy services, social work, psychological therapies as well.

Case Management
Each individual is assigned a case manager who assists individuals with identifying and articulating priority outcomes, personal goals, and needs for services and supports. The case manager will advocate for consumer rights and benefits in court settings when needed. The case manager will ensure that good rapport is established with individuals and their family members
Support Services
Our compassionate team will give direct personal assistance with activities of daily living (grooming, eating, bathing, dressing, and personal hygiene). Assistance with meal planning and preparation, securing and providing transportation, assistance with house keeping, assistance with abulation and mobility, reinformcement of counseling and therapy activities, and much more.
Health Services
Health services include nursing services, physical therapy, occupational therapy, speech and language pathology, audiology, social work, behavioral support, and dietary services.
Residential Services
This service is provided in a small group home setting with 24-hour awake residential support staff who provide services and supports the needs of the individual and is present in the residence and awake whenever an individual is present in the residence. The resident support staff is assigned on a daily shift schedule that includes at least one complete change of provider staff each day.
